Read and review all of the Policies of Grow Family Health
Print and Sign/Initial the Summary Page. This confirms that you have reviewed, understood and that you agree to comply with the policies outlined.
Print, Choose Payment Option, Sign
Option 1: Pay-per-use with deposit​
Submit the "Uninsured Services Option" form to confirm your choice.
Pay the deposit online (click here) or in person​​
Option 2: Pay-per-use with credit card on file​
Submit the "Uninsured Services Option" form to confirm your choice.
Submit the "Card Authorization" form.
Come into the office to add credit card details to your secure Square profile.​​
Option 3: Pre-payment (bundled services discount)​
Submit the "Uninsured Services Option" form to confirm your choice.
Submit payment online (click here) or in person.​​
Bring all completed documents to Prisma Healthcare Collaborative/Grow Clinic 320 Adelaide Street S.

Give completed forms to our front desk staff and ask for a Ministry of Health form. Fill out this form prior to leaving the clinic and return to front desk.
